GUIDELINES

1. Purpose

These guidelines implement the procedures for the recognition of initiatives and the recognition of application effectiveness, replicability and scope of impact of initiatives/ science, technology and innovation tasks at Tra Vinh General Hospital, ensuring compliance with the regulations and requirements of the Vinh Long Provincial Department of Health.

2. Applicable Subjects

Authors and co-authors of initiatives/ science, technology and innovation tasks.

4.1. At the Institutional Level: The application dossier for evaluation and recognition of the application effectiveness, replicability and scope of impact of initiatives and science, technology and innovation tasks at the institutional level shall be submitted to the Provincial Department of Health and include: 

(1) A written request from the head of the institution for recognition of the application effectiveness, replicability and scope of impact of the initiative or science, technology and innovation task at the institution, using Form No. 07/SK-VBĐN. 

(2) The author's report on the application effectiveness, replicability and scope of impact of the initiative or science, technology and innovation task, using Form No. 08/SK-BCHQ. 

(3) For initiatives: the Initiative Certificate (Form No. 03/SK.GCN) issued by the institution recognizing the initiative in accordance with regulations on initiatives; for science, technology and innovation tasks: the Certificate of Registration of the Results of the science, technology and innovation task in accordance with current regulations. 

(4) Supporting documents demonstrating the application, replicability and scope of impact of the initiative or science, technology and innovation task and its practical benefits to production, society and management; other relevant documents and products such as images, records, physical models and actual products that can be provided (if any)

4.2. At the Provincial and National Levels: The application dossier for evaluation and recognition of the application effectiveness, replicability and scope of impact of initiatives and science, technology and innovation tasks at the provincial or national level shall be submitted to the Provincial Department of Science and Technology and include: 

(1) A written request from the Provincial Department of Health for evaluation and recognition of the application effectiveness, replicability and scope of impact of the initiative or science, technology and innovation task at the provincial or national level. 

(2) The author's report on the application effectiveness, replicability and scope of impact of the initiative or science, technology and innovation task. 

(3) A document certifying or confirming the application effectiveness and scope of impact of the initiative or science, technology and innovation task at the institution, issued by the head of the agency, organization or unit authorized to confer the title of Grassroots Emulation Fighter, using Form No. 13 in the Appendix to Decree No. 152/2025/NĐ CP. 

(4) Supporting documents demonstrating the application, replicability and scope of impact of the initiative or science, technology and innovation task and its practical benefits to production, society and management; other relevant documents and products such as images, records, physical models and actual products that can be provided (if any).

- For initiatives: the Initiative Certificate issued by the institution recognizing the initiative in accordance with Appendix III issued together with Circular No. 18/2013/TT-BKHCN dated 01/8/2013 of the Ministry of Science and Technology guiding the implementation of certain provisions of the Regulation on Initiatives issued under Government Decree No. 13/2012/NĐ-CP dated 02/3/2012 

- For science, technology and innovation tasks: the acceptance decision of the competent agency or organization; the Certificate of Registration of the task results in accordance with regulations. 

- A decision or document confirming the application effectiveness and scope of impact of the initiative/science, technology and innovation task at the institution, issued by the head of the agency, organization or unit authorized to confer the title of "Grassroots Emulation Fighter", using Form No. 13 in the Appendix to Government Decree No. 152/2025/NĐ CP dated 14/6/2025.

4. Legal Basis 

• Pursuant to the Law on Science, Technology and Innovation dated 27/6/2025; Pursuant to the Law on Emulation and Commendation dated 15/6/2022; 
• Pursuant to Government Decree No. 13/2012/NĐ-CP dated 02/3/2012 promulgating the Regulation on Initiatives; 
• Pursuant to Government Decree No. 152/2025/NĐ-CP dated 14/6/2025 on decentralization and delegation of authority in the field of emulation and commendation, detailing and guiding the implementation of certain articles of the Law on Emulation and Commendation; 
• Pursuant to Government Decree No. 129/2025/NĐ-CP dated 11/6/2025 on the delineation of authority of two-level local governments in the state management areas of the Ministry of Home Affairs; 
• Pursuant to Circular No. 18/2013/TT-BKHCN dated 01/8/2013 of the Minister of Science and Technology guiding the implementation of certain provisions of the Regulation on Initiatives issued under Decree No. 13/2012/NĐ-CP; 
• Pursuant to Circular No. 15/2025/TT-BNV dated 04/08/2025 of the Ministry of Home Affairs prescribing measures to organize and guide the implementation of the Law on Emulation and Commendation and Government Decree No. 152/2025/NĐ-CP dated 14/6/2025 on decentralization and delegation of authority in the field of emulation and commendation, detailing and guiding the implementation of certain articles of the Law on Emulation and Commendation; 
• Pursuant to Resolution No. 190/2019/NQ-HĐND dated 12/7/2019 of the Vinh Long Provincial People's Council prescribing the contents and expenditure levels from the state budget for initiative activities in Vinh Long Province; 
• Pursuant to Resolution No. 16/2025/NQHĐND dated September 10, 2025 of the Vinh Long Provincial People's Council promulgating the Regulation on contents and expenditure levels for science and technology tasks in Vinh Long Province; 
• Pursuant to Decision No. 299/2025/QĐ-UBND dated 12/11/2025 of the Provincial People's Committee promulgating the Regulation on evaluation and recognition of the application effectiveness, replicability and scope of impact of initiatives and science, technology and innovation tasks in Vinh Long Province; 
• Pursuant to Decision No. 2440/QĐ-UBND dated 05/11/2025 of the Provincial People's Committee on the application of legal normative documents of the Provincial People's Committee after the merger; 
• Pursuant to Official Letter No. 1943/SKHCN-ĐMST dated 05/11/2025 of the Provincial Department of Science and Technology guiding the recognition of initiatives and the recognition of application effectiveness, replicability and scope of impact of initiatives and science, technology and innovation tasks; 
• Pursuant to Guideline No. 01/HD-SKHCN dated 24/11/2025 of the Provincial Department of Science and Technology on implementing certain articles of Decision No. 299/2025/QĐ-UBND dated 12/11/2025 of the Provincial People's Committee promulgating the Regulation on evaluation and recognition of the application effectiveness, replicability and scope of impact of initiatives and science, technology and innovation tasks in Vinh Long Province.
